From condition to closure —
without the blank page

NEXUS Corrective Action opens with a front door anyone in the plant can use in under a minute — and behind it, the discipline your program is graded on: guided significance, causal analysis that gets the code right, and closure that stands on evidence.

THE PROBLEM WE BUILT FOR

Corrective action tools fail at both ends: a front door so heavy nobody reports, and determinations so blank that quality depends on who filled in the box.

NEXUS removes work instead of adding fields. Reporting takes less than a minute. And the hard determinations arrive structured — the record captures what was considered, not just what was concluded — so two engineers with the same facts finally produce the same kind of record.

THE HARD TRUTH ABOUT TREND DATA

Improvement follows the codes — even when the codes are wrong

Every program trends its cause codes. Almost nobody trusts them — because coding happens at the end of a long day, the taxonomy is a wall of jargon, and "human error" is always the path of least resistance. Miscoded causes bend the trend chart away from what's truly happening in the system, and improvement money follows the chart — unequal improvement, aimed at the wrong problems.

NEXUS makes the right code the easy code: a proven, public-domain causal tree with plain-language definitions and examples at the point of coding, the method enforced where it matters — a human-performance code must name the condition it's coupled with — and overrides recorded with reasons, so even the disagreements become signal.

The payoff shows up in Clarity: when the codes are honest, the trend a leader sees is the system as it is — and the improvement investment finally lands where the problems actually live.

WHAT IT COVERS

The corrective-action lifecycle, governed

01

A front door with no gatekeeper

Condition reports from anyone — phone, kiosk, QR — description-only minimum, anonymous option. "I saw something" to "it's on the record" in under a minute.

Under a minuteMobile / kiosk / QRAnonymous option
02

Guided significance, graded approach

A short risk walk suggests the grade with its rationale shown; the screener accepts or overrides with a recorded reason.

03

Cause codes that tell the truth

A proven causal tree with definitions at the point of coding — bare "human error" is never an accepted answer; the coupled condition must be named.

04

Extent and effectiveness, structured

Extent-of-condition worked as an enumerated review; effectiveness criteria fixed at approval; closure stands on the evidence rather than outrunning it.

05

Nonconformance & MRB

Four dispositions with required technical justification, regulatory clocks that know happened-vs-known, and a cost-of-quality ledger leadership can read.

06

Closed means closed

Records never quietly reopen — a failed fix opens a successor at escalated significance with a permanent tie-back. The genealogy auditors wish every system kept.

THE QUIET DIFFERENCE

Rigor you can hand to an auditor

CONSIDERED, NOT JUST CONCLUDED

The record shows the scope examined and the reasoning used — a determination an outsider can follow, years later.

INSUFFICIENT DATA SAYS SO

Where evidence can't support a call, the record says exactly that and escalates — never a quiet default to "fine."

FINDINGS FLOW IN, WHOLE

Inspection failures and oversight findings arrive as governed conditions with their evidence attached — one corrective lifecycle for the whole platform.

If the top bar is "human error," we should talk. We'll walk one real event through guided coding and show you what your trend chart has been hiding.
